Just how long in the internal timeline has Sam been wearing that cast now? I mean, he says it's been four weeks since Ava, and he's had it for several eps before that now.
Actually I've never broken a bone myself, and I don't know how long a casts has to stay on usually, but do we need to assume that Sam reinjured it at some point by now or something because he keeps using it? (and obviously I realize that in reality it's because of external factors such as the actor being injured, but I cling to my intra-universe viewpoint)

When I did my wrist in as an eleven-year-old, I was in a cast for about four weeks but, being eleven, I healed very quickly. For my ankles at about the same age, I was in casts for about six weeks. More recent breaks - spine, skull and multiple collar-bones - have been of the sort where you don't get a cast.
Why, yes, I am very accident-prone.

Yeah, when I broke my arm as a kid I had to wear it in a sling for about 6 weeks, but it wasn't a very bad break. Maybe Sam broke more than one bone in his hand, or something.

But yeah, I think Sam mst have had the cast longer than six weeks at least.

Perhaps it is taking a long time to heal because Jared can't simply rest it as a normal person would? Unfortunately, he has to keep using it in the course of his work. It's just a guess on my part.
I find it very hard to figure out the internal timeline of SPN episodes, I'm of the opinon that TPTB keep it deliberately vague.
